For an initial contact with a college, I would recommend creating a profile page. Have 3 sections, Contact Information: your name; address; telephone; e-mail address. Athletic Information: primary and secondary positions; height; weight; bat/throw (left/right); High School Head coach's name and phone#; summer coach's name & phone number; personal baseball statistics. Academic Information: High School attending; guidance counselor name and phone number; Grade Point Average; Year of Graduation; SAT/ACT scores and date taken; registered with Clearinghouse (Y/N).
Send the profile with a cover letter expressing why you are interested in their school and baseball program. Make sure to personalize the letter with the coach's name. Let them know of any tournaments or showcases you may be attending, etc. Attach the profile and include your high school schedule if college is local.
I hope this helps you get started. Follow the suggested recruiting timeline. If you are a junior or a senior I would suggest following up the letter with a phone call within 1 week.
