ONLY when he is throwing as hard as he can???? That would a problem I think...Anytime you have pain when throwing that is a signal the body is giving him to say something is wrong...IMO he needs to stop what he's doing and figure out what is causing the pain...Is it mechanical? Did it just start or has it been going on for awhile and he is only now admitting it? What is the pain on a scale of 1-10? Is his velocity down recently? Address the pain and figure out why it is happening. Ignoring it and getting on the mound is about the worst thing you can let him do...

Patient:  Doctor, my shoulder hurts when I throw the baseball as hard as I can.

 

Doctor:  Well, stop throwing the baseball for a month and see if that helps.
