Of all the dozens of pitchers I know that have dealt with this issue I don’t know one that didn’t eventually have TJ surgery. If they wanted to keep playing. IMO UCL injuries just don’t heal any other way. Even a “sprain” is damage and further throwing once it’s detected always seems to lead to more damage until surgery is the only alternative.
