Without seeing a video of you it is hard to diagnose, but I would say make sure your hands stay close together and don't "catch" the ball. Use the glove as a mechanism to "deflect" the ball into the throwing hand.

To practice use a paddle glove or a 9.5" small glove.
Yeah a video would be great to see and really figure out what's going on.

Always remember that you want to give the SS a target with your hands. Position your hands with palms facing outward toward the SS. Make sure the thumbs are together. Hopefully the throw is perfect to the chest but if not the hands should be able to lead the body / feet toward the throw.

I'm going to take a guess on what might be a problem is you are catching the throw and then transferring to throwing hand. Don't do that if this happens to be the problem. The throw from the SS should hit your palm with your throwing hand wrapping / trapping the ball for the transfer.

Some people will call this deflecting or transferring or something else. What you call it isn't what's important but the action is. The best way I've heard it explained and the way I explain it now is the ball never stops moving. The glove and throwing hand are only there to help redirect the ball to a new direction. When the ball goes into the pocket then that movement is now stopped.

When you field do you use the palm push or funneling method? The first two things however that come to mind is either you dont get your top hand over the glove or you have very small hands. As mentioned by others though without video its hard to say.
