Is it true that the high school rule for coach conferences is 3, and the coach must pull the pitcher on every visit after the third? (visit counting as one where the coach does not pull the pitcher, when the pitcher is pulled this does not count as a visit)

Coach gets three charged conferences per game. A charged conference is one in which the pitch is not replaced. After three, the pitcher is pulled at each succeeding conference.

In extra innings, the coach gets one charged conference per inning.

That conference is charged when the coach crosses the foul line and talks to any defensive player, not just the pitcher. The conference ends when the coach steps back over the foul line. If they go into extra innings, the defense get one conference per extra inning. BTW, the offense gets one conference per inning.

How many in one inning to have to pull pitcher?

So 3 no pull but on 4th visit have to pull?

Correct -- all visits could even be with the same batter.

NCAA is a combination of FED and OBR: 3 per game (plus one more if the game goes extra innings), but also the pitcher must be removed on the second visit in an inning.
