This issue can be quite complicated, and the following is only a rough description. The easiest situation is NCAA DIII. The NCAA only requires that a player's academic progress be acceptable to the college.
NAIA schools need 24 hours in the two previous semesters, and 48 transferable hours at the beginning of the 3rd year of participation (not the 3rd year of enrollment).
DII requires 24 transferable hours in the two previous semesters, or an average of 12 hours per term of attendance. Starting the 3rd year of enrollment in college, the only hours that count are ones which can be used toward the students degree program.
D1 is quite difficult for a player in your situation. A player starting their 4th year of enrollment needs to have completed 60% of the credit hours required for his declared degree program. For many schools that corresponds to 72 semester hours. It isn't enough to just have transferable hours; the credits from the 3rd year on have to fit into the degree program.
