Terminology in recruiting questionnaire

I realize this may fall into the "if you have to ask..." category, but here goes anyway.

I notice that some of the colleges have recruiting questionaires on their website and some refer to basecall america, perfect game, and team one, with a blank to fill in beside each of these. Some also have a line for ST report, and a Reg report, with a blank for these.

Are they looking for a rating from the first three? Also, I don't know what the ST and Reg reports mean. Anyone care to enlighten me?
