Tommy John Symptoms
I'm kind of new to this site so I searched I search "Tommy John" and read through the forums. So it seems like a lot of people here have some experience on this subject. First off I'm 17 and a senior in HS. I've been having arm issues lately and I have a doctors appointment scheduled for next week. Partially Torn UCL right before senior year
Hey guys, After seeing two doctors, the second being Dr. Ciccotti (Phillies), I have been diagnosed with a partially torn UCL. I still have a dynamic ultrasound test next week to determine the severity of the tear but just wanted some opinions. I'm a pitcher going into my senior year of college. This past season is when I started dealing with elbow pain but I pitched through the season and took a couple weeks off to see how it was but the pain came back even worse a few weeks later after 2...

UCL Primary Repair
Does anyone have experience with UCL Primary Repair with backing (UPR) versus Tommy John surgery (TJ)? Do scouts think UPL is more of a red flag than TJ? I know TJ has a long track record and UPR is relatively new but there appears to be at least 3 MLB and several dozen MiLB players that have had UPL and a higher success rate.

Trojans35, I'm not a doctor, but I have researched elbow injuries a bit due to my son's issues last year. The symptoms you list do sound consistent with a UCL injury. Get to that doctor's appointment. Try to find the best, closest sports injury physician working with elbow issues that you can. The physician will do a physical exam asking you to press against his hand in different positions, and move your arm in various ways to look for obvious tears. I don't think anyone would want to give you wrong advice here, so definitely see a doctor as soon as possible. If it can be arranged, it is best to jump right to someone who handles athletes - or at the least an orthopedist. Your typical family doctor will most likely just prescribe rest for a few weeks before passing you along to a specialist. You can probably save some time by skipping that step. All good advice above, especially the suggestion to locate a doc with specific experience diagnosing and treating throwing athletes. MRI with contrast is typically a good first step.

Our son had Tommy John two years ago. He felt the problem right away, on a specific pitch. Said it felt like hitting his funny bone, but in his elbow. MRI did not show the tear, but after an attempt at rest and rehab, he had the surgery in which the doctor (Dodgers doc) did a few tests in his office, said it was easy to diagnose and after the surgery said there was extensive damage. Advice above is good - get to a doctor who knows what he/she is doing on elbow injuries.

My son's UCL tear was described as long term and progressive. He never felt a pop. But in the last week of the summer he did experience a significant velo drop that got worse every outing, and then eventually mild pain while throwing. Ditto all the advice above, plus this - despite all you hear about it within the baseball community, TJ is actually a fairly rare procedure in the overall population. Did Dr Ciccotti talk to you about the "Primary Repair" technique? I have read that it significantly reduces rehab time, but the surgeon won't know for sure whether or not you're a candidate until the scope is in your elbow. Thanks! He didn't discuss surgery much since hes hoping I can rehab it but I'll bring it up to him at my follow up appointment in 2 weeks. Although if I have the surgery in August, 6-7 months recovery would bring me back during the first month of our season, it may be worth considering.

Based upon orthopedic advice, my son sat out 6 weeks at the end of his senior HS season. He then rehabbed from approx. May 20 thru July 10. Got to college, nothing hurt, per se but his velocity was down 3-4 MPH and then slowly dropped further until he called and said elbow was hurting again. Had to have TJ surgery. Was told PRP would make it feel better but that in the Ortho's opinion (Kremchek), it was a short term fix and son would be right back in 5-6 month needing TJ surgery. He elected...

Thanks for sharing your experience! That's exactly what I don't want to happen and it adds on to the similar stories I've heard. Obviously I wont make any final decisions until I see my doctor again and discuss with him but I'm leaning towards surgery because with grad school as an option it seems to give me the best chance to play my 4th year.

I'm so sorry to hear this. I'm impressed with your boy's attitude so far, though. Must be a testament to your love and care for him. I hope he stays part of the team, if he wants to, and that whatever decision he makes, the results are kinder to him.

ETR, So sorry to hear about this. An injury is never easy to deal with. A "re-injury" just seems cruel. I wish I could offer sage advice, but you are in rare territory with a re-injury and your son seems to have a more than reasonable plan to address it. When my son was injured after his sophomore year all he could think about (besides school) was getting back on the field, and competing again. It was strong motivation. ETR, Such a difficult turn of events. Baseball can be so very, very cruel at times. We're dealing with an injury right now too so I have some idea of what you're going through. I wouldn't even want to think about doing it twice. Just when you think you are on a path, it sometimes takes a bad turn. ETR, as the dad of a pitcher that has been through the non-surgical rehab for a UCL injury as a HS sophomore, then nerve transposition surgery in the fall of his freshman season only to end up having TJ surgery that same December, I can somewhat empathize with you. My son's recovery from the TJ was not the "smooth" path that so many attest to. He is 26 months post surgery. I will tell you there are days he calls me and says, "Dad, my arm is bothering me again." It's just words that crush...

Sorry to hear this, best of luck to your son and him getting healthy. Here's another perspective he should seriously consider. There is more to life than baseball. My son played for 4 years, was injured and lost most of a season, came back and finished and had a nice Sr year. He went on and got his masters (fully paid because of summer internships) and now has a nice life working for a rapidly growing tech company. Thanks to everyone for your kind and supportive words. Younggun, I am wishing your son all the best and thinking good thoughts for him every step of the way. Listening to your story certainly brings back memories with our son and the multiple physical setbacks. I feel your pain but I feel a strong sense of calm with the way you describe your/his scenario. It is obvious that your son is balanced and well grounded and will adjust well to these changes in life that will continue to present themselves long after baseball days. I totally feel for your son, and your family. I agree with others that it is definitely a cruel twist of fate, but from your story, I believe your son is going to make it! Is your son a candidate for internal brace repair? See if he can be referred to Dr. Dugas or Dr. Poletta. Faster comeback. May not be possible with an existing graft, but all options are worth exploring.

ETR, My heart just sank reading this.....hang in there. My son had shoulder surgery last year and for a baseball kid and a baseball family it's rough . It's also absolutely impossible to fully understand unless you've been through it. But to go thru major surgery and endure the rigors of rehab only to discover that another surgery was necessary ? ETR as a Father of a kid who had surgery last year I can empathize with you totally. I blamed myself and was angry at others about it. But I thought I'd share our experience for what it's worth if it maybe helps. My 2019 will be 6 months post Internal Brace Repair surgery by Dr. Dugas on March 8th. Thanks to all for sharing. The medical consensus seems to be that damage to son's graft is extensive and unfortnately a full revision is needed. He will have it done next Friday. They will use the hamstring rather than the palmaris this time. He has a strong support system in place for the rehab and at this point just wants to get it done and start moving forward.

The first outing should be about how does he physically feel while pitching and how does he feel the next day. Everything else will follow over time.

PT, pens, working out, instruction, strength training, mental preparation, etc., can all be very important pieces of a successful comeback. That said, live game adrenaline is an entirely different element. It's a bit like early in the PT process. Fortunately, it doesn't take nearly as long to ramp back up.

Tommy John first outing
My son had Tommy John surgery 15 months ago and had his first outing this last weekend. There were a lot of nerves And a lot of excitement. He struck the first person out but then kind of fell apart after that walking 2 and 2 hits. It was tough to watch but he persevered. After the game he was very upset with his performance and said he wasn’t sure if he could do it anymore. My question to the group is do you have any stories about Tommy John comebacks and their first outings? (Good or bad)...

My son’s first outing was almost 12 months to day from injury and 11.5 months from surgery. His team was very careful with him. His first two outing he faced 3 batters and those were a week apart. He was originally scheduled to be a starter that year but coaches ended up leaving him in bullpen and longest outing was 3 innings that year. RJM is correct in that it should be about how he felt physically. I know the trainer was all over my son for first month after each outing checking him out...

I haven't worked with any "post Tommy John" surgery patients, but from my experience, almost everybody has some mental issues in trusting their body parts after surgery. As stated above, Follow the advice of the surgeon and the Physical Therapist, and time( and reps) will play a large part in getting the "trust" back (so that he doesn't have to think about anything except the next pitch). My son's experience is similar so I would encourage your son and tell him it gets better. My son's first few outings he had mechanical issues, didn't have a lot of feel, was timid to let it go... he made it through the year but wasn't really sharp. He struggled with arm slot and a few mechanical changes that happened with his delivery. Year two is when he really felt better. Baseballin, buckle up as this is gonna be long. Sorry up front. Son had TJ surgery in Nov 2015. This was his freshman season at LSU. The plan was to redshirt and then he should be more than ready for spring 2017. During fall workouts of 2016 he did very little throwing to the best of my recollection. My son came back 11.5 months after surgery in 2016 and 1st outing went well and the rest of the season not good. In retrospect came back too early, velocity was there, control was not, nor was recovery time. He would state that the toughest thing wasn't the surgery and rehab it was "sucking " that year. Got better every year and is still playing in the minors chasing his dream.

Son is in year 2 of return now. Had TJ early 2018. Summer 2019 pens went from 86 to touching 90. Fall 2019 / spring 2020 velocity inconsistent 86-92 and control inconsistent. The day he left campus when the season was shut down (March 2020) he touched 93 for the first time. Continued being inconsistent until he went to Driveline in July 2020. By November 2020 he was sitting 93 touching 95 but still inconsistent control. This season his control is good but has had little setbacks (intercostal...
