R1 & R3 RHP from stretch makes an inside move, ends up facing 2nd, no throw. Neither runner did anything other than take a normal lead. No clue as to what pitcher thought he would accomplish?

Did his foot hit the ground behind the mound? Or, did he deliver a pitch but turned toward 2B while doing it?
Balk probably in all codes if he actually made a pickoff attempt to 2B. He wasn't driving a runner back for FED. And, there was no judgment by the umpire that the runner at 1B was making a legitimate attempt for 2B.

He ended up straddling the rubber, ball in hand, facing 2nd base.
Sounds like he forgot it was 1st and 3rd instead of 1st and 2nd. Balk.
