R1 & R3 RHP from stretch makes an inside move, ends up facing 2nd, no throw. Neither runner did anything other than take a normal lead. No clue as to what pitcher thought he would accomplish?

Did his foot hit the ground behind the mound? Or, did he deliver a pitch but turned toward 2B while doing it?

Balk probably in all codes if he actually made a pickoff attempt to 2B. He wasn't driving a runner back for FED. And, there was no judgment by the umpire that the runner at 1B was making a legitimate attempt for 2B.

So, I have a balk.
