Yankee - Bellarmine University in Louisville, Kentucky fits most of your criteria.
I have no idea how successful they have been this year in baseball - they have had a new coach. I'm sure you could go to their website and gather some info. In the past they have recruited in Louisville or fairly close to home. They are a DII.
Very nice campus in a fairly wealthy part of Louisville. Easy access off the interstate. Field is on campus. Might want to check it out.
Carthage college in Wisconsin. I don't know if its catholic but its a very very good D 3 school always ranked, good academics. One negative thing though: they have like 40 guys on a roster, so that means not much playing time.
St. Josephs in Indiana is a great school. I recently went to their spring training camp over valentines day weekend. Good facilities and great coaching. Coach O'Dette is great. they are a solid acedimic school. avg. GPA for incoming freshmen (on a 4.0) is a 3.18. I would check them out.
A summer teammate of my son is a freshman at Briarcliff, a Catholic University in Iowa www.briarcliff.edu. It is NAIA, but the competition level seems to be similar to NCAA D2 or D3. It sounds like it offers good academics (this player is a very good student and received both athletic and academic money).
He seems very happy there, except for playing time. But he's a freshman and that's to be expected. He likes the school a lot and he's working his behind off. We like that!
In the Midwest, 2 very good baseball programs are St. Thomas and St. Olafs in Minnesota. Both have had a number of players drafted or signed as free agents. St Thomas is a perennial NCAA Regional team and St Olaf's would be but they have to beat St. Thomas.
If you want to go to a school in the north east i would say Franklin Pierce College its a D2 in NH that is rapidly getting strong has potentially best D2 baseball player in the nation.
Are you still looking? Not quite the Mid-West, but in the Mid-South is Christian Brothers University...they have a baseball player from St. Paul, a graduate of Cretin-Derham Hall, or is it Derham Hall-Cretin, I'm confused.
My son goes there and LOVES it. D2, excellent conference, players from many parts of the nation. Good luck.
Alvernia College, D-III Catholic college in PA. Small college but growing. Baseball team is ranked 7th pre-season this yr in Collegiate Baseball poll and 2nd on Baseball America pre-season poll. Finished 9th place in post season polls last yr. Coach is up front w/kids about their playing time in the fall so they know what they need to work on over the winter. Kids get along great on the team.
