Runner on second, left hand pitcher in the stretch. Pitcher comes set and on his second look to second base does a backside spin to turn and throw but trips after the spin and falls down no throw is made. In a spin move to second is he required to throw?

No throw required but he does need to step toward the base (distance and direction).

Also remember, just because it's ugly doesn't mean it's a balk.

Agreed. I've seen non-balks that were both ugly and rife with deception.
