Is it the player's uniform number on the lineup turned in to the umpire that is used to determine if a player batted out of order. For example: If the lineup had Charlie batting 3rd and had his uniform number is listed as 15 on the lineup, but the number on his uniform was 25, is he batting out of order when he bats third wearing the uniform number 25 instead of 15?

The name is what matters.

The unreported substitue becomes legal when he enters the batter's box. (unless, of course he was an illegal player for other reason...re-entry vioation, ejected, etc.)

Reporting does not legitimize a player.
