I don't see anything wrong with it. I'd keep it simple. Introduce yourself, say how old your are, what year in HS, what position, and why you're interested in their school. Then see where it goes. Be ready to send a dvd or an email with a link to a video. You could ask about camps or other places that he could see you play.

He may ask you to go to their website and fill out the recruiting questionairre. It may not be a bad idea to do that before calling so you can show that you are already being pro-active.
good point about filling the questionnaire out ahead of time. also have sent your stuff in. cold calling is ok to an extent as long as you keep it brief. just mention to the coach that you are interested in their program. tell him you sent him a schedule or you are sending him a schedule and you hope he can make it out to see you play. he'll take over. if he's busy it will be brief. coaches stay busy. they're in constant contact with guys they're already recruiting. once you've made a phone call then you sit back and collect feedback. see if you get any mail. see if he comes to the games. if not they aren't interested.
