1 per inning for the offense.
3 per 7-inning game for the defense (no conference charged if the pitcher is removed) 1 per inning for extra innings. Any additional defensive conferences require removal of the pitcher.

If either team has a charged conference, the other team can conference (and not be charged) as long as it breaks up when the charged conference ends.

Ok you have a starting pitcher and you make a trip in the first, second and third inning. So you make the pitching change in the third inning - now do you have another three trips since it's a new pitcher OR do you have to make a change on your first trip (fourth overall) to the new pitcher?

I have always wondered about this.

You get 3 trips per GAME. After the 3rd charged conference, every visit requires a pitching change.

In your example, the 3rd visit wouldn't be charged because you changed the pitcher. You would still have one left.
