Let me make sure I'm right here. You have Jones as a DH for Smith. Later, you have Smith bat.

At this point, the DH is removed from the game and Smith becomes the batting spot for that position.

Fed 3-1-4a: The role of the designated hitter is terminated for the remainder of the game when:
a. the defensive player, or any previous defensive player for whom the designated hitter batted, subsequently bats, pinch-hits or pinch-runs for the designated hitter

From NFHS Rule 3-1-3:
ART. 3 Any of the starting players may be withdrawn and re-entered once, including a player who was the designated hitter, provided such player occupies the same batting position whenever he is in the lineup. .... A substitute who is withdrawn may not re-enter.

So Player B can't re-enter, no matter who he substituted for.

In this scenario Player B becomes the DH when he subs for Player A. If Player A re-enters for Player B he returns as the DH and Player B is done for the game.

I had the privelege of coaching in a league that played major league rules with modifications 2 out could run for pitcher or catcher intentional walk just send him down played 7 innings. they went to the Federation rules about 8 or 9 years ago.
