In a recent game the opposing coach turned in a line up card and had the starting pitcher listed in the #4 slot as P/DH.  The pitcher pitched 2 innings and then was replaced by another pitcher, yet he remained in the game as the DH.  The umpire stated that was legal as it was submitted that way on the card.  It was my understanding that a defensive player could not occupy a position in the field and the DH at the same time.  The umpire said it was a pitcher specific exception.  Is this correct?

In NCAA rules, yes.

No in all others.
