I know that any runs scored after what would have been out #3 are not counted as earned against a pitcher.  Rule 10.16 (MLB) does not clarify, however, if the error is made on what would have been the second out.  Example: 1 out, man on first, and the batter gets on base on a fielding error by the first baseman.  The next batter is struck out, for out #2.  Then a triple scores both runners.  Are those both unearned runs, is the first run earned and the second not, or are the both earned?  Thanks.

No earned runs in your scenario. You always reconstruct the inning had their been no fielding or throwing errors and no passed balls by the catcher. In reconstructing your scenario, the error happens when the second out should have happened. Reconstruct it thus- runner on first, one out. Next batter grounds out to first for second out advancing runner to second, two outs now with runner at second. Next batter strikes out for third out, runner stranded at second base, no runs score.
