Live ball:
Any player, not a coach, may ask for an appeal.The ball does not have to go to the mound as many believe but can just be thrown to the base being appealed or to the base that the runner now occupies. In OBR if the takes off and they play on him then the appeal is nullified. If the ball does go to the mound and stays live the pitcher can throw to any base without disengaging.
Dead ball:
The ball must go to the mound and PROPERLY be put in play. If the pitcher does not have the ball everyone thinks he does, the CAN NOT be made live. The reason for this distinction is the pitcher cannot be called for a balk for being on the rubber without the ball nor can the runner be called out for being tagged by a fielder trying the hidden ball trick.
I realize that is way more info than asked but I have seen this screwed up so many times by coaches and umpires alike.
