Runner on first. Right handed pitcher goes into stretch. Runner takes lead. Pitcher picks up front leg and runner goes. Pitcher wheels, makes pick-off turn to second base, and throws the runner out. This is not a balk. It is an especially effective tool when there is a 3-2 count with two outs.

He's OK as long as he does it in one motion without stopping or "hanging the leg."

Would he be able to fake a throw to second if the runner is going?

Sure. He can do just about anything to 2nd - throw, fake a throw, or throw to the shortstop at his normal position. He just has to step to 2nd if he's on the rubber.
