In all rules codes, the ball is always a dead when a pitch hits the batter. The hands are part of the batter, not the bat.
Yes, as Dash said, the ball hits the batter it is dead. If he isn't swinging it is a HBP unless it is in the strike zone.

I follow what you've said so far, but what is it ruled if it is in the strike zone? Does that depend on what happens to the ball (i.e. foul, etc..)?

As was said the hands are part of the batter so just ask yourself "what happens if the batter sticks his knee or elbow into the zone and gets hit?" The answer of course is a dead ball and a strike and if its strike three the batter is out. "What happens to the ball" doesn't matter here.

Strikes are trump. If the pitch is in the strike zone, or if the batter swings at the pitch, it is always a strike even if the batter is hit by the pitch..
