Pitch Tracking Sheet

Does anyone have a good pitch tracking sheet that they keep on their pitchers during games? I have one that I use, just wanted to see if anyone uses anything better.

Always looking to help out our pitchers and players the best I can.

does any one use point streaks auto scoring - gamesheet system - our league is using - great idea, but what a problem trying to get all to submit scoresheets. great for tracking balls and strikes, keeping stats for palyers. Scoring hits/errors is the biggest issue, each sees the game differntly.
