How much of the pitcher's foot has to be in contact with the front edge of the pitchers plate? Is it the same for both the stretch and windup (once the pitcher has pivoted)? Is this rule universal or different at different levels?

In FED only, from the set position only, the ENTIRE pivot foot must be in contact with, or directly in front of, the pitcher's plate. I don't go looking for this though. Some of the mounds have a crater in front of the rubber from being dug out (and not properly maintained), and the pitcher is not looking for an advantage, he's just looking for some level ground for his pivot foot.
