I would personally suggest that all questionaires be filled out. Sometimes it doesn't matter, but unless you are a blue chip player, I would leave all of my options open.

There are also usually areas on the questionaire to recommend another player , you might be able to help a fellow teammate or friend get a look because of your help!

Without a doubt fill it out!!! College Coaches talk. They always seem more interested in a player when they know other schools want that same player

The other point which I learned right here on Hsbbweb is that you never want to burn bridges You never know where a coach may end up. Now, if you become more actively recruited, I think it's best not to lead the coach on and waste his time. If you're absolutely sure you have no interest in going there, that is.
