Here's a quirky one from the fellas at FED:

Bases loaded, no outs. B4 hits an infield fly and ump calls B4 out. R3, thinking ball will not be caught, advances past R2 just beyond 2nd base. The ball then hits R2 right on the helmet while he is
a) standing on 2nd
b) he is off 2nd...

That is the problem question on this year's Fed test. It is poorly worded.
