There are some tips under the
Recruiting Tips section of the homepage:
Sample Letters. In general, keep it short, just a 1 page intro will do; personalize it (avoid a "dear coach" address); thank them for their interest in you as a student/athlete; let them know how interested you are in their program; if they haven't sent you one of their player profile forms to fill out, then attach one of your own (samples can be found on the link above); provide contact info (e-mail & home address & telephone#) below your signature; and finally attach any additional info on where they can see you play, such as a HS schedule or a schedule of your summer showcase plans.
Hope this is helpful. Don't get too concerned about formats. The main thing is to make contact with the coach and let them know of your interest. Good Luck!
