Both an error and a sac fly. As far as the batter goes score it a sac fly. The batter is not charged an AB and gets the RBI. (he did good) ---- On the defense score an error on the outfielder. (they did bad) This satisfies the scorebook as far as the batter/runner reaching base on the error and should he score --- it would result in an unearned run.

Same thing with slight variation. R3 1 out, deep fly ball but runner did not tag thinking it would drop in and did not score. We scored it as an out with no sac, just a case where a runner messes up a batter. (which are many) In your case I am not sure but think I would score it as a Sac with no RBI.
