Can someone explain the scoring of a balk created by the catcher leaving the catchers box prior to the pitch being delivered? I'm certain that the balk is charged, by definition, to the pitcher, but how is it noted in the scorebook?

In 44 seasons, at all levels, I called this balk ONCE. Then I spent five minutes explaining the rule to both teams, and both sets of bleachers.
