I agree you should go to the doctor and get an excuse from school....this will also excuse you from all school activities. This protects you from unnecessary criticism from the coach or any rules you might violate.
Call the coach and explain your situation. This will impress upon the coach that you have desire and commitment and are serious about making the team.
Let me give you an example of how adversity can work for you. For 15 years I was a service technician with IBM. My job was to troubleshoot and solve customer’s problems with their IBM equipment. Some problems were very complex and could take days to resolve. Many times I felt as if the customers were ready to chew me out and throw me out the door ---but---- surprisingly these were the customers that would recognize my commitment and desire to solve their problems. These are the customers that would take the time to write a letter to my manager thanking me for my desire to solve their problem. ..... So, remember it’s not always what you accomplish that gets recognized, but your desire and commitment too!

Unless someone unexpected comes in, the coach knows who his players will be and I'd guess that he has a good idea what his starting lineup will be on opening day.

Be sure to that him know of your status, I'm sure if you have what it takes, he'll keep you in mind. You just don't want him to think you decieded not to play.
