I've seen plenty of bonehead pitching in my time. #1 on my list of pitching No No's is manning the mound without adequate warm up.

Pro teams universally take the warmup seriously, but HS coaches and players often don't. Ten tosses isn't a warmup!

Share your warmup comments and routine. Does your team have a fairly precise number of required bull pen tosses?

I am pretty sure my son strained his lat due to improper warm up. Not that he didn't warm up properly, but needs to warm up longer due to colder weather in SC.
One of hardest adjustments is being a warm weather pitcher all of your life nd trying to make the transition to colder weather. (30-40 degree days).

I once saw the pitching staff do sprints and jumping jacks for 20 minutes. Then the starter went to the mound for about 5 pre-game tosses Jeesh! That was on a team for 12 year olds tho.

Our pregame warm up includes 1 hr of stretching,running,tubing long toss. Well conditioned pitchers generally only need 25-30 pitches. Start infront of mound for a few and then back of mond for a few then throw 10 moderate throws and then 5-6 throws at near max. 3-4 main pitches. If you aren't ready by then you never will be.
Cold weather may require more. All your warnups are thrown without a jacket which I see pitchers doing occasionally.
Gotta go with Bobble here. We send our pitches to the pen 25 minutes before game time. Thats after stretching and lite running. They then go thru a front and back of the mound warm-up and then 30+ pitches. We do let them go hard when they're ready. Also gotta remember the regular weekly regiment of pen work, long toss, flat work, running, tubing, lifting. We gotta prepare these kids to pitch, then they'll warm up faster.
