I can't help with an offensive wristband because I don't have an example of one on me, but go here to view one that is used for the pitcher/catcher:

P.S. Most of the people who use my software use a code very similar to this:

H&R - Hit and Run
SAC - Sacrifice Bunt
S - Steal
DS - Double Steal
BFH - Bunt for Hit
___ - Dummy Sign

You can make it as complicated as you want and add more signals like bunt to 1st, bunt to 3rd, run and hit, 1st and 3rd plays, etc. but I think it's more practical when coaches don't over-do it and have more than 10 offensive signals. I've seen some coaches that use 15 different signals and I think that is just way too much, but, there are different coaching styles! I just prefer to keep it simple!
For the last 3 year I have created my own wristbands using excel sheet. Every time I redo them it takes me approx. 2 hours to change them in order to get it done, proof it, and feel comfortable that they are random. I bought the program from Xan and let me just say it is well worth the money. To top it off when I had issues with it he gave me his phone number and walked me through it. Some of the best money I have ever spent.
