If there is runners on 1st and 3rd, how much of a fake do you have to do to make it legal to throw to first after you have lifted your leg? Also once you fake to third can you throw to second if the runner takes off?

On the first to third move a step is all that is needed. In HS rules you may step back and throw to first while still engaged. All other codes require you be dis-engaged whether you throw or fake to first.
On the second question, you may throw to second if you do one of two things. Either step and throw to second or disengage and then throw, with a step or not.
