The other day in a JV game I think I saw an opportunity for the fourth out benefitting the defensive team.

Home team had two on with two outs and the batter slammed ball to the fence in right center. The first and second runners easily scored although the second runner clearly missed third base. The batter was thrown out trying to stretch the hit into a triple for the supposed third out.

My questions are:
As the tag was made at third, the fielders for the most part just took off toward their dugout thinking that they were lucky they at least got the hitter out. How do you go about appealing this play on the second base runner or is it too late at a certain point(maybe when several players reach the dugout?)

I believe this would save a run if the second runner is appealed but I'm curious how this would be scored. My guess is a one RBI double if the second runner is out on appeal.

You are correct on your assessment of the runner's run being nullified upon appeal. An appeal in this case has to occur before the pitcher and infielders have left fair territory (8-2 Penalty.)
Let's say typical high school kids and they don't know about the potential appeal at third but the coach saw it. Third out is made on the tag and they start heading off the field. Two or three have made it to foul territory but not the dugout.

Can the coach tell his team to stay on the field and then ask for the appeal once they enter back into fair territory?

Can the coach send a player(s) back onto the field to ask for the appeal if they had reached the dugout before the coach asks?

As long as there is still the pitcher or any infielder still in fair territory, the appeal (live-ball or dead-ball) can be made.

To be clear, as long as all infielders had not left fair territory, the appeal can be made. This does not include a fielder that exited fair territory and then subsequently returned to make the appeal.

The part of the appeal process on which I was unclear was exactly what yall addressed--was it still alright to appeal as long as someone such as the pitcher or another infielder was still out on the infield. Many times its the coach who sees the missed base and in this case he wouldn't have been able to stop some of his infielders in time before they were off the infield but probably could have stopped a couple and the pitcher in time. Thanks a lot.
