R1, one out. Batter hits a bounding ball to F6. F6 bobbles the ball and has no play on either runner. F6 then realizes that the BR rounded first and attempts to retire him but throws the ball out of play.
Where to place runners?

Two bases from time of throw, so R1 gets home and BR gets third.

Correct in all codes. The "first play by an infielder" rule doesn't apply in this situation (see OBR 7.05g APPROVED RULING).

APPROVED RULING: If all runners, including the batter-runner, have advanced at least one base when an infielder makes a wild throw on the first play after the pitch, the award shall be governed by the position of the runners when the wild throw was made.

Since this is a high school website, the pertinent FED rule is the penultimate sentence of 8-5. "If every runner, including the batter-runner, has advanced one base at the time of the first play, the award is two bases from the time of the throw."

Unless you're reading the Fed books; then it's the other way around.

Sometimes...and that's the real problem. R1 could be on any base depending on the number of runners.
