I was wondering if anyone knows colleges that have online questionnaires relativley D1-D2 Baseball Colleges. I live in NJ but they dont have to be in NJ but im mainly looking for eastcoast.

Best thing to do is go to the college website and go to the baseball page. I would say it's a safe bet that quite a few schools now have some sort of online survey / questionaire.
But even if they don't there will be phone numbers, email addresses and physical mailing address.
The thing to do is make some sort of contact with the coaches. Try to find out if they have camps or showcases where they will be at so you can try to be seen by them. Don't expect them to come find you - you have to go find them.
Plus, go after schools where you have a realistic shot at playing. For example, if you have D3 talent then don't try to get on at North Carolina. Find some good D3 schools and contact them. Just keep your talent level realistic and if your not sure where you are find someone who is reputable and ask them to evaluate you.
