The most effective and important pitch to control and master is the fastball. A fastball followed by a change up makes the pitch that much slower and a change-up followed by a fastball makes it that much faster. In short, the fast ball needs to be controlled. Like Coachric said, its mechanical or mental and I'll lean toward the mental part. As a righty pitcher, i tend to throw outside to lefty batters so therefor i invision a righty instead and that allows me to re-gain control. I would suggest during pitching practice at school work soley on your fastball untill you can locate/throw for strikes. Unless you would like a career of 3 years due to too many curve balls or a BP change-up, i suggest emphasis on working at it.
As you draw back, I see your wrist curling around the ball. This creates tension in your hand and forearm that will rob velocity and also make it hard to get your index and middle fingers directly behind the ball at release. The result is that you may turn your fastball over one time, cut it the next, so that you don't have consistency with your spot control.

There have been pitchers who have thrown with cupping like this with some success. Rick Sutcliffe and Kent Mercker come to mind. However, for most of us, it can be a hindrance.

Try loosening up your wrist, then work at getting the ball up in back, facing second base, before you start your arm forward. Eliminate the tension, let your hand and the ball whip out, keep your fingers straight through the ball (i.e., if you want control and velocity, don't cut the pitch every time). Work on having your fingers pull through the center of the ball with downward, not sideways, action. You will know when you're doing this when your catcher sees 12/6 backspin on the pitch coming in.

Cupping is used by some with off-speed pitches and so this suspicion of the source of your fastball problem is consistent with your saying that you are not having difficulties with your curve and change. But at this point this is just one suggestion for something you could tinker with on a trial-and-error basis, not something you could be positive about until you see how a change affects things.

One drill you can do for this is the sock drill a/k/a the towel drill. Place a chair facing you, in front of where your stride foot lands. Hold a sock or towel in your throwing hand. Go through your throwing motion and work at having the sock or towel extend to hit the seat of the chair. If your wrist and finger action are correct, you can do this consistently. If you are twisting the ball at release, the sock or towel will knot up and miss the chair.

Hope this helps!
