Well, IMO, from the limited info and only seeing one swing, if you have said your elbow is staying close to your body, i don't see how you are getting jammed.  Some full video of more than one swing may help those on here that are more adapt in the hitting dept help you out.  It may be as simple as doing some T drills to help you concentrate on your contact point

 

looks like your hand are getting involved too early..so you are not creating enough separation from your lower half. you want your hips opening when front foot lands and hands come last...looks like your hands are creeping forward and following your shoulder path rather than staying back and driving toward the ball

You are turning very fast and leaving the bat too far behind you.  You are probably hitting a lot off handle because the bat is lagged and it is tipping as you pull around with your shoulders.  Get a feel of the bat in your hand and feel some extension out front.
