Ground ball to short but he airmails to first way over first baseman but up the line (towards home). The runner is two feet from first when the fielder attempting to catch the bad throw knocks the runner down before he steps on the bag. Ball goes off the fence and back to the first baseman who tags the runner out. Umpire rules out at first.

In a lot of these, as Mike says, they turn out to be a "HTBT",......2 feet from the bag is clearly a point where the ball, the runner and the fielder all can occupy the same space and not result in anything to be called.....this is what we refer to as a "trainwreck"...

Your last post would have me leaning toward nothing more than a train wreck....play on...
