7 year old pitching machine game, first baseman fields ground ball, falls down, and his cap comes off, he grabs his hat and touches first with it prior to the batter runner reaching first, out or safe? I say out.

Safe. The cap is detached player equipment. For this to be an out, the fielder's head would have to be in the cap.

is the hat considered any less a part of the uniform/equipment than the cleats? His foot never touches the bag, his cleats do, if his cleat comes off and he has his foot half way in it and touches the bag with the cleat, is the runner out, or is it considered detached equipment?
No mention of the hand touching the bag. Why would he describe the touch as being with the hat if the hand touched the bag? You seem to be stretching a bit. Of course, if I was caught screwing up a rule that bad, I'd probably be trying to extricate myself as well.
