Coach gets three charged conferences per game. A charged conference is one in which the pitch is not replaced. After three, the pitcher is pulled at each succeeding conference.

In extra innings, the coach gets one charged conference per inning.

Correct -- all visits could even be with the same batter.

 

NCAA is a combination of FED and OBR:  3 per game (plus one more if the game goes extra innings), but also the pitcher must be removed on the second visit in an inning.
