Just watched the Phillies' pitcher launch a throw over the first baseman's head. Got me thinking.

What play has the highest percentage of errors? My guess would be the pitcher throwing to first and even second base after a comebacker.

Didn't the Rays lose a playoff series in large part due to pitcher's throwing errors?

I'd disagree with that one. If the shortstop has to range far and then make an off balance throw, it's generally ruled a hit. Also, from that distance, the 1b has a lot of time to move off the bag and get in front of the throw.

It's an interesting question though. The ones to first from a charging 3b would seem to have a high incidence of error. Short distance = not much reaction time, and the 3b trying to get a lot on the throw and usually throwing from down under.

Great question. I would agree with you that that play seems to get screwed up the most often.

from what Ive seen in the minors if the ss gets to the ball (no matter how hard the play) and makes a throw the 1st baseman cant handle then its error on the ss.
I think my top 5 plays that have the highest error percentage are

1. Pitcher making any throw after fielding his position

2. 3B charging and throwing across body on bunts / slow rollers

3. Pitcher covering 1B when ball gets in hole between first and second where second baseman makes the play

4. Catcher fielding / throwing a bunt up the right side pretty much on the line

5. Base coaches trying to catch foul grounders with one hand
