Lots of problems, but the main one you need to fix first is you are sliding forward with your hips. Hit behind your front leg and sit on your back leg. Watch some video on this website of MLB hitters and look at their contact position. Focus on the front and back leg. Master that and then you can start working on your swing.
Hands go from your shoulder and drop down onto the ball... there's no power there... Your hands should move back slightly to gain momentum, then forward...
Hips should rotate in slightly, then fire through... your hips aren't really rotating properly...
You also need to work on your footwork, on some swings at impact your back heel is on the ground... some you're in a decent position, but this needs to happen on every swing... consistency!
You seem to have decent eye-hand coordination, but there's a lot of things going on that you need to work on... Find a good instructor in your area and work with them...
This is a case where you should be broken down by a good hittinbg instructor and built back up. It wouldn't be as difficult as it may sound, but you need a new approach to hitting. Try speaking with a hitting instructor and going from there. Best of luck to you.
Wow, it's terrible. Sorry, but this is bad... Lots of things wrong. Out in front, falling off, you're late, etc... The bat also looks like it is too heavy for you. No offense, but you need some lessons if you want to take this seriously. Good luck.
