I had almost the same problem as you when i was a freshman. I pitch and play outfield and my arm wasn't strong at all. I would have to say the thing that helped me out the most was long tossing and using weighted balls. I would long toss and use the heavy balls around 3-5 times a week and by the time next season came around my arm improved a lot. You should really try it out.
I added 5 mph using some of Nolanr Ryan's techniques. You do NOT WANT TO PUSH off the rubber, it adds tons of stress to your elbow. You will want to use the tall and fall technique. Stay balanced and go into a relaxed, controlled fall towards home, but stay balanced. This, along with proper mechanics and conditioning will help you find the answers to your problems.
