This happened in a HS girls' softball game in our area yesterday. Runner on third, no one out. Batter flys out to left field, runner on third tags up and is thrown out at the plate. EVERYONE in the park thinks there's three outs and both teams switch up. First batter doubles. Coach for the defensive team finally brings the fact that only two outs were recorded the previous half inning to the ump's attention. He orders the defensive team back to the plate where the third out is recorded on a ground out. The ump then allows the batting team to begin their inning with a runner on second (from the lead-off double in the "false start" inning). Everyone follow that? My opinion is that they should have started the inning fresh, with the batter who doubled before leading off the inning. What's everyone else think?

This is true...the events had absolutely no bearing on the game's outcome. I think I just would have done it differently. Of course the losing coach played the game under protest even when, in my eyes, it was HIS team that benefitted from the call. It was HIS player who doubled and was able to take second base at the beginning of the "real" inning.
I had a similar situation except that I had a runner on first base too. The difference was that I was able to call R1 out for abandoning the bases once he took his place in the field, so I didn't have to worry about bringing one team back in to hit for 1 more out and then switching places again after that out.

It is a very odd situation and I think the umpire handled it well, I just may have denied the defense's request that they be permitted to return to bat after they willingly went to the field with only 2 outs. I wouldn't enjoy the conversation the next inning when we tried to determine who should lead off for the short-changed team.

How would you other veterans have handled this?

As the ump, I would have changed nothing!. If both teams and the umps all thought there were 3 outs, and the teams changed positions and started the half inning, its too late to go back.--Similar to the defensive coach in the 6th inning showing that there was an illegal batter in the 3rd (who'se no longer in the game)--too late, too bad.
