Southern CA has several options for playing a fairly competitive 19U baseball schedule during the spring for kids who aren't playing for a HS for whatever reason. Is this common/uncommon elsewhere?

The only option that I'm aware of in my region for high school aged kids during the high school season is Jr/Sr Little League, and I think a number of those kids also play for their high schools.

Yes they do. I've yet to hear of any skipping HS baseball. Best talent comparison is HS JV. One guy we had started playing at age 16, no way he was going to make a school team. A fair number of pitchers who would be riding the bench quit and joined the league. A couple older guys were hoping to walk-on in college. Even a few middle school kids playing up. I believe most areas have rules about playing concurrently on a HS team, if not, HS coaches would forbid it either way.

I think most of it around here is second chance for kids that don't make their HS team and home school kids.  The teams play around at different private high schools.  It can get competitive to make a roster at larger high schools.

I don't think that any HS players are in on it though.

My 2012 played in the SoCal "alternate" high school league in Spring 2012. Talent level was pretty stellar on some teams (30+ scouts at several games and a lot of future pro and D1 talent) to other teams with high school JV level players and college cuts. It was not limited to 19u. Age range was roughly 16-21. Weekend games only and no practices. My 2012 was doing a gap year due to his young age / maturity.

I believe that is a money maker for the Pecos league. I knew at one point, if a player played in another Spring league , they could not play on the Pecos League.
