The practice and playing season are shown in Section 17 of each of the NCAA division manuals. See especially Figure 17.2 for D1/D2 and Fig. 17.1 for D3.

Section 17.2 in each manual has info relating specifically to baseball, and that's where you'll see the 45 day D1 limitation and different limitation in the other divisions.

Also, the NCAA defines practice, skill instruction (17.1.6.2.3), conditioning (17.1.6.2.x), and "voluntary" activities (17.02.13). We tend to lump these all together as "practice", but the rules treat them separately. I think you'll find that any D1 baseball player "practices" a lot more than 45 days in the fall.

There is no specific dates designated for fall practice, lot depends of geographical areas. Colder weather teams might do field practice first, then more on conditioning and strength training after the colder weather sets in, warmer climate schools will do the reverse.
There is a time frame for spring practice to begin on the field, for D1 implemented a few years ago. This prevents warmer weather programs t get in more field practice than colder climate programs.
3 Fingerglove nailed it (as usual). Son's D1 school is on the semester system, starts 8/31. Fall ball starts the 2nd week of school and lasts for the allowed 45 days. At that point the team starts their 4 on 1's as Bobblehead stated.

Those on the quarter system generally start later in Sept. and will start their fall ball accordingly.
