First Question:
With a runner only on 1st base, can a pitcher fake a throw to third base then turn back and try and catch the runner off 1st base?

Second Question:
For a pick off move towards 2nd base, can the pitcher leg lift with hand separation with a spin towards 2nd or will the leg lift and hand separation be counted as a movement towards home plate?

yes, I didnt see only a runner at first. That is correct you cant fake to 3rd if no one there. I skipped ahead, because we were called for a balk on that play with runners at 1st AND 3rd. The umpire swore he played in the minors and that was a balk. His justification was a lefty can't fake to 1st, so a righty cant fake to 3rd. It really didn't matter we lost the discussion even though I knew he was wrong.

Not the same situation, that was definately a balk in your case.

The owner of the one recognized pro-schools has said that former pro players are the hardest to educate and train to become umpires. By that level, player and coach induced myths are really hard to dispel.
