PatrickW,

These are the books that I regularly use (some I've had since I was in the 8th grade, others I've recently added):

Instructional:
The Act of Pitching- Dr. John Bagonzi
Nolan Ryan's Pitcher's Bible- Nolan Ryan and Tom House
Coaching Pitchers- Joe McFarland
The Pitching Edge- Tom House
The Louisville Sluggers Complete Book of Pitching- Doug Myers and Mark Gola

Mental Aspect of Pitching:
Heads-Up Baseball- Tom Hanson Ken Ravizza
The Mental ABC's of Pitching-H.A. Dorfman
The Mental Game of Baseball Dorfman and Karl Kuehl

Hope this helps!
If you had to pick one I would recommend the Louisville book. It has all the progression shots of top pitchers and looks at all the styles ,pitch types food selection, correcting problems and a complete work out section. My son usually takes it with him every where he plays. I think Chapters has it for $14 and it is the best all around book I have read.
For videos, we use the following for reference in our program:

Pitching Mechanics - Jerry Weinstein (Rockies Organization)
Developing Pitches - Jerry Weinstein (Rockies Organization)
Thrive On Throwing - Alan Jaeger (Long Toss Program)
4 DVD Set - Chris Sinacori (Mariners Organization)
3 Tape Set - U of Miami (Chair Drills)

These videos have brought organization into our in-between starts workouts, focus and improvement on our mechanics, and brought our mental and fielding skills up.
