My son (Sophomore) received a questionnaire froma D1 school. Once we send it in, do we follow up to make sure they received it? I know in the letter they say they can not have contact with him until after September 1 of his Junior year. Also do we ask the colleges that he is interested in for questionnaires or are they just sent out by the colleges?

If there are schools that you are interested in, you should, by all means, write the coach and ask for information on the program and the college itself. Typically, if they write back, they will include a questionnaire. They are very limited in the contact they can have with a sophomore, and the rules on what type of contact they can have and when they can have it are detailed at the NCAA's website.
