High school & summer team info is all that is important. Recruiters aren't interested in pre-HS. Include these catagories:
HS contact info:
School name, address & phone; Counselor name, email & phone; Coaches name(s), email, & cell phone.

Academic:
SAT / ACT scores; GPA (core courses); class rank;
awards, (like Nat'l Honor Society, Merit Finalist, Math Olympiad, Debate Team, etc)

HS Athletic:
Position(s); Awards by year

Extra Curricular: Clubs, volunteer work, leadership activities, etc.

Summer Team:
Coaches Names, email, & phones
Positions Awards; Showcases, Significant Tournaments (WWBA, Aflac,etc).

Current stats:
60 yd, pitching velocity, pop time, etc.

References:
Son listed the pro scouts w/phone numbers who had seen him, PG contacts, etc.
