When I coached, we had the batter step out and rub his eye like he had dirt in it.  I know it seems like a lot, but we wanted to make sure he had the sign.   As far as a back pick at first, a lot of times our 1B would call it based on what he saw from the baserunner on the play before  ex) the kid was really slow getting back to the bag after the pitch.   He would say "need a strike here 12" or whatever number the pitcher was.  If the catcher was calling it he would say "need a strike here here Mallory" or  whatever the 1B's sister's or girlfriend's name was.  Nobody we played against knew what the heck we were talking about so it worked out good.

For a couple of seasons our batter was supposed to  spit to confirm that he saw my suicide sign.

BTW,  our batter is giving the confirmation sign to me, not to our runner, because I don't trust our runner to pick up on it.

If the batter doesn't confirm, I tell  R3,  "Suicide is off."
